When July 25th – August 4th
Where: YLHS Stadium
Time: 10am-1pm
Cost: $250
REGISTRATION STEPS:
1. Complete registration through FamilyID: Click Here
2. Complete and turn in Physical and Insurance card to the main office.
3. Make payment for summer camp (Check, Cashier’s Check or Money Order Only)
4. Complete the form on this page (For Board to order proper shirt sizes)
Where/How do I take care of Physical
Every athelete is required to complete a physical each year.
There are two options to complete the physical –
1. Visit your regular docter, simply click here to download the form, complete it, have the doctor complete their portion and then return it to the main office.
2. Attend the Physical Night May 24th @ Esperanza – Only $35.00
Physical will be completed and they take care of providing to district
How do I make payment for summer camp
TWO WAYS TO MAKE PAYMENT FOR SUMMER CAMP:
- By Mail: (Check, Cashier’s Check or Money Orders Only)
- Print completed registration from Family ID and attach payment (No Cash)
- Please ensure that on your check, cashier’s check or money order, there is:
- A phone number where we can reach you with any questions
- Name of your child the payment is for
- The school name and sport
- In Person (preferred): (Check, Cashiers Check or Money Order Only)
- Bring in copy of completed Family ID registration
- Attach payment (No Cash)
